• Summary of Benefits and Coverage (SBC): Provides a snapshot of what your health insurance plan covers, and the costs associated with it.
• Evidence of Coverage (EOC): Offers a detailed, comprehensive explanation of your plan's coverage, rules, and member rights. It's the full legal contract between you and Health Net.
